Document Your COVID-19 Out of Province Immunization Record(s)

Who Does This Apply To

• Anyone who lives in the City of Toronto and received any doses of Covid vaccine outside Ontario.
• Anyone who lives in the City of Toronto and received any doses of Covid vaccine from the federal (non-Ontario stock) vaccine supply. Generally these doses were delivered at institutions such as correctional facilities, military sites or embassies.

These doses should be documented to create a complete Ontario vaccination record.

What Do I Need To Complete This Process

• You will be required to share your personal information and City of Toronto home address  
• You will be required to submit a valid immunization record or receipt for each out of province or non-Ontario stock dose. If you do not have the record(s) or receipt(s), please request it from the health care provider(s) that immunized you.
• You will also be required to provide a phone number and/or email address so we can reach you to inform you that your record(s) has been created.
